Job Description

JOB OVERVIEW

The Family Stability Case Manager position works with American Indian families with children under the age of 18 living in Hennepin County and are in the early stages of family instability. The position emphasizes advocacy for children and families, works to establish supportive relationships, and empowers families to build on their strengths. Click HERE to learn more.

 

RESPONSIBILITIES

  • Conduct intake with new clients, assess needs and short-term goals, and create case plans
  • Refer clients to chemical health programs and assessments, treatment centers, methadone or other programs, recovery support services as well as cultural, social, educational, and employment services
  • Transport clients to appointments using company van or personal car.
  • Work collaboratively as a member of the Early Intervention and Family Services staff team
  • Provide intensive case management with each individual client on your case load as well as providing assistance to other clients when needed
  • Conduct home visits; Participate in occasional supervised visits between the parents and children
  • Maintain client confidentiality
  • Assist in conducting program evaluation forms
  • Maintain and keep timely case note records, and correspondence with service providers when requested
  • Maintain statistical data and provide semi-annual reports to the Early Intervention Programs Supervisor
  • Attend monthly staff meetings and participate in other assigned meetings and community events
  • Various duties as assigned by Early Intervention Programs Supervisor and/or Family Services Program Director

 

QUALIFICATIONS

  • Bachelor’s degree or multiple years of social service experience
  • Some Case Management experience
  • Knowledge of Child Development, addiction, and experience working with American Indian families
  • Knowledge of community resources and recovery support services
  • Strong desire to assist in providing holistic and cultural services to clients
  • Excellent communication, written, and computer skills
  • Knowledge of the Native American community and its needs
  • Knowledge of the Data Privacy Policies for social services agencies
  • Possesses excellent communication, written, and computer skills

 

REQUIREMENTS

  • Complete and pass criminal records and driving record check
  • Reliable transportation to drive clients to appointments
  • Maintain a valid Minnesota driver’s license and current auto insurance
  • No drug, alcohol, or substance abuse related problems
  • Schedule: Mon – Fri, 8:30 am – 4:30 pm
