PROGRAM DESCRIPTION
The Bright Beginnings Recovery Support Project supports Native American women who are pregnant or have recently delivered, and who have a history of substance abuse, and previous experience with the child protection system. Click HERE to learn more.
RESPONSIBILITIES
- Conduct intake with new clients, assess needs and short-term goals, and create a plan for case management
- Refer clients to chemical health programs and assessments, treatment centers, methadone or other programs, recovery support services as well as cultural, social, educational, and employment services
- Transport clients to appointments using company van or personal car
- Work collaboratively as a member of both the Bright Beginnings Project and Family Services staff team.
- Provide intensive case management with each individual client on your case load and to other clients when needed
- Conduct home visits and occasional supervised visits between parents and children
- Make presentations along with Bright Beginnings Supervisor and/or other staff to other Native American agencies, hospitals, clinics, doula programs, etc.
- Maintain client confidentiality
- Assist in conducting program evaluation forms, groups, etc. along with Bright Beginnings Supervisor
Maintain and keep timely case note records, and correspondence with service providers when requested
Maintain statistical data and provide quarterly reports to the Bright Beginnings Supervisor
Attend monthly staff meetings and participate in other assigned meetings and community events
Various duties as assigned by the Bright Beginnings Supervisor and/or the Family Services Program Director
QUALIFICATIONS
- Bachelor’s degree or multiple years of social service experience.
- Some Case Management Experience
- Knowledge of community resources and recovery support services.
- Strong desire to assist in providing holistic and cultural services to clients.
- Knowledge of the Native American community and its needs
- Strong desire to work in a team environment and jointly provide groups to participants.
- Knowledge of the Data Privacy Policies for social services agencies
- Excellent communication, written, and computer skills.
REQUIREMENTS
- Complete and pass criminal records and driving record check
- Reliable transportation to drive clients to appointments
- Maintain a valid Minnesota driver’s license and current auto insurance
- No drug, alcohol, or substance abuse related problems
- Schedule: Mon – Fri, 8:30 am – 4:30 pm
